People arrive unhappy with filler placed somewhere else. It has migrated, or there is more of it than they agreed to, or it sits in lumps they can feel, or it is simply not the shape they asked for. What almost all of them have in common is that they have already been offered more filler to balance it out.
Sometimes that is genuinely the right answer. Often it is not, and it is worth being blunt about why: adding product to a result you dislike usually produces more of a result you dislike, with the added problem that there is now more material in the tissue to deal with later.
No urgency here.
The question is not what to add. It is what is already in there.

WHAT WE DID
Working Out What Is Actually There
The first obstacle is information. Most people do not know what was used, how much, exactly where, or how long ago, and that decides everything. Hyaluronic acid can be broken down with hyaluronidase. Calcium hydroxylapatite, poly L lactic acid, permanent fillers such as silicone, and fat transfer cannot. If you have records, bring them; if you do not, say so, because it changes what is realistically possible rather than ending the conversation.
The second is that not everything blamed on filler is filler. Swelling, natural asymmetry that was always there, weight change and normal ageing all get attributed to an injection, and treating something that is not there does nobody any good.
Then the honest part about dissolving itself. It causes swelling, and the area can look deflated once the product goes and before your own tissue settles, particularly where filler has been in place for a long time. Hyaluronidase also acts on your own hyaluronic acid temporarily, which contributes to that. It is not the final result, but it is several days you need to plan for, and it is the reason nobody should book this in the week before an event.
Afterwards there is a wait before anything is replaced, both because hyaluronidase stays active in the tissue for a period and because nobody can assess what an area needs while it is still swollen.

What to Expect, and Who It Is For
What the Appointment Looks Like
- Bring whatever records you have. Product names, dates, areas. Anything is better than nothing.
- Allergy history, properly. Hyaluronidase comes from an animal source, so this is asked for a reason.
- An honest answer about what can be reversed. Including when the answer is that it cannot.
- Plan for looking worse before better. Several days of swelling and a deflated look.
- A wait before anything is replaced. The interval depends on how much was dissolved.
- A fresh assessment afterwards. Decided on what is actually there once things have settled.
Who This Applies To, and Who It Does Not
This is for anyone living with a filler result they did not want, and for anyone who has been told repeatedly that the answer is more product.
- It only works on hyaluronic acid. Calcium hydroxylapatite, poly L lactic acid, permanent fillers and fat transfer are not reversible this way.
- If you genuinely do not know what was injected, that changes what can be attempted and how cautiously.
- Do not book it in the fortnight before an event. The swelling is the point at which people panic.
- Allergy history matters here more than in most treatments, particularly to bee or wasp stings.
One exception overrides all of this. If filler is causing a vascular occlusion, that is a medical emergency and it is treated immediately.

Questions People Ask in This Situation
Straight answers, the same ones you would get in the treatment room.
- What if I do not know what was injected?
- It is a very common position to be in and it does not end the conversation, but it does change it. We work from what the area looks like, how it feels, when it was done and how it has behaved since, and that usually points strongly toward one material or another. What it means in practice is proceeding more cautiously, because the confidence you would get from a product name is not there.
- Does hyaluronidase dissolve my own hyaluronic acid too?
- Temporarily, yes, and that is part of why an area can look flatter than you expected in the days afterwards. Your own hyaluronic acid is continuously replaced, so this is a short lived effect rather than a permanent loss. It is still worth knowing in advance, because otherwise the first few days are alarming for no good reason.
- How long before I can have filler again?
- Not the same day, and usually not the same week. Hyaluronidase remains active in the tissue for a period, so filler placed too soon can be partly broken down before it settles, and the swelling has to resolve before anyone can judge what the area actually needs. You will be given a specific interval based on how much was dissolved and where.
- Will I look deflated afterwards?
- Very likely for a few days, and you should plan around it rather than hope. It is most pronounced where filler had been in place a long time, because your own tissue has adapted around it. It is not the final result. Book it when you have a quiet week rather than before something you care about.
- Can everything be reversed?
- No, and this is the most important thing to understand before anyone injects anything into your face. Only hyaluronic acid can be dissolved. Calcium hydroxylapatite, collagen stimulators, permanent fillers and fat transfer break down on their own timelines or not at all. It is the main reason we prefer hyaluronic acid for most purposes: not because it lasts longest, but because it can be undone.
