How Dermal Fillers Work

Dermal fillers are gel formulations of hyaluronic acid, a substance your skin already produces, injected beneath the skin to restore lost volume and support facial structure. They can lift cheeks, soften nasolabial folds, refine the jawline, or add subtle definition with lip filler.

Because the cheeks move far less than the lips, cheek filler usually lasts longer, often twelve to eighteen months or more, and we build it in stages so the result stays in proportion with the rest of your face.

At Promethean, filler placement is mapped to your bone structure and the specific areas where you have lost volume, not to a standard, symmetrical template. We inject conservatively in stages: a subtle result can always be built on, but overfilled tissue takes months to settle and cannot be immediately undone (hyaluronidase can reverse hyaluronic acid filler if ever needed).

What to Expect

  1. Consultation and assessment. A review of your facial structure, your goals, and where volume loss is actually occurring, with an honest opinion on whether filler is the right tool.
  2. A written plan. The exact areas, the product, the estimated volume, and the cost, quoted in writing before anything is injected.
  3. The treatment. Thirty to forty five minutes depending on the areas treated. A topical numbing cream is applied first for comfort.
  4. Right after. Mild swelling or bruising is normal and usually settles within a week. You can return to most normal activities the same day, avoiding strenuous exercise for 24 hours.
  5. The follow up. A two to three week review to assess the settled result, with any conservative refinement only if it genuinely improves the outcome.

Do you offer payment plans or financing?
We go over the full cost in writing at your consultation, before any syringes are booked. If a payment plan or financing option would help, we are happy to discuss what is available at that time, with no obligation to decide on the spot.

What happens at the consultation, and how long does it take?

Who performs the injections?
Your injector is an aesthetic physician or a registered nurse injector on our nurse practitioner led, physician supported team. Every treatment at Promethean is planned as medicine first.

What is the serious risk with filler, and what happens if it occurs?
The one that matters is vascular occlusion, where filler presses on or enters a blood vessel and restricts blood supply. It is rare, and it is the reason filler belongs with a trained medical injector rather than anywhere cheaper. The warning signs are immediate: blanching or a dusky, mottled look to the skin, and pain out of proportion to the injection. It is treated urgently with hyaluronidase, which dissolves hyaluronic acid filler. We keep it on site.
Do you use a needle or a cannula?
Both, chosen by area. A cannula is blunt tipped and flexible, and in areas with significant vessels it is more likely to push a vessel aside than to pierce it, which lowers the risk of occlusion without removing it. A fine needle gives more precision in places where placement has to be exact. The choice is a clinical one for your anatomy, and we will explain which we are using and why.
How long does filler last, and does it depend where it goes?
It depends far more on the area than on the brand. Filler in a mobile area such as the lips is broken down faster by constant movement, and typically holds for around six to twelve months. Filler placed deeper, in the cheeks or along the jaw, sits in a quieter area and often lasts twelve to eighteen months or longer. Metabolism, exercise level and how much was placed all shift that.
Can filler migrate, and can it be removed?
Hyaluronic acid filler can move from where it was placed, most visibly in the lips, and it is more likely with large volumes, repeated top ups before the previous product has gone, or placement in the wrong plane. It is also the reason hyaluronic acid is the material we prefer: it can be dissolved with hyaluronidase if you are unhappy or if a correction is needed. Not every filler on the market is reversible.
What should I tell you before filler?
Tell us about blood thinners, aspirin, ibuprofen and fish oil, which all raise the chance of bruising. Tell us about any history of cold sores, because injecting around the mouth can trigger an outbreak and it can be prevented. Tell us about autoimmune conditions, recent dental work, and any filler you have had before, including what, where and when, even if it was years ago and somewhere else.
