People often arrive with a unit count rather than a concern. I had thirty two last time. My friend gets forty. It is completely understandable, because units are the one part of this treatment that looks easy to compare between clinics, and it is the wrong thing to anchor on. A unit measures how much product was used. It does not measure how good the result was.

The number that suited you somewhere else was calculated for the muscles you had at the time, by someone assessing a face that has since been treated repeatedly. Carrying that figure forward as a target rather than as history is one of the most common routes to being over treated, and the person it happens to usually asked for it.

Why the Number Went Down Instead of Up

The assessment looks at the face twice, at rest and moving, because a still face hides the thing being treated. Lines that appear only when you frown behave differently from lines etched in at rest, and only the first kind responds fully to a neurotoxin. Asking for more product will not change that.

Muscles treated consistently for years often do less work than they used to. That is the treatment succeeding. It also means the dose that was right at the start can be more than is needed later, so a falling unit count in a long term patient is frequently a sign that things have gone well rather than that someone is short changing you.

Over treating has a cost that is not obvious until it arrives. Shut one muscle group down too firmly and its neighbours compensate, which is where a heavy brow or an unfamiliar new line comes from. Restraint here is not caution for its own sake. It is how the result carries on looking like a face.

So a first plan with us is deliberately conservative, with a review two weeks later. Adding is a five minute conversation. Subtracting is a wait of months.

What to Expect, and Who It Is For

What the Appointment Looks Like

  1. Bring your history, not your target. What you have had, roughly when, and what you liked or did not like about it.
  2. Assessed at rest and in animation. You will be asked to frown, lift your brows and squint.
  3. The number in writing, with the reasoning. Areas, dose and cost, before anything is injected.
  4. Conservative on the first pass. Deliberately, because it establishes how your muscles respond.
  5. A two week review. Where the result is judged, and where a dose is added if it is genuinely short.

Who This Applies To, and Who It Does Not

This is for anyone who has been treated before and suspects they are being sold a number rather than assessed, and for anyone whose results have started to look heavier than they wanted.

  • If your lines are etched in at rest, a neurotoxin will soften rather than erase them, and more units will not change that. Something else is the answer.
  • If your last result faded unusually fast, the cause is more often too small a dose, or treatments booked so close together that the previous one never cleared, than a bad product.
  • If what you actually want is no movement at all, we are not the clinic for that.

The goal is the smallest dose that achieves what you came in for, not the largest one you would have paid for.

Questions People Ask in This Situation

Straight answers, the same ones you would get in the treatment room.

Does fewer units mean I am getting less for my money?
You pay for the product used, so fewer units costs less. The better question is what result you are buying. A larger dose that leaves you with a heavy brow is not better value, it is a worse outcome you paid more for. Judge it on whether your face does what you wanted it to do, not on the quantity on the invoice.

Why did another clinic use more?
There are several honest possibilities. Your muscles genuinely were stronger then. The assessment was done differently. Or the plan was built around a package rather than around your face. We cannot tell you which, and we are not going to speculate about another clinician. What we can do is show you the reasoning behind our number and let you judge it.

Does long term treatment mean I need less over time?
Often, yes. Muscles treated consistently tend to weaken over the years, which is rather the point, so a dose that was appropriate at the start can become more than necessary. It is not universal, and it is reassessed at every visit rather than assumed in either direction.

What if it wears off sooner than I expected?
Say so, because the fix depends on the cause. Genuine underdosing is corrected by adjusting the plan next time. Treatments booked too close together, where the previous dose never fully cleared, is a spacing problem, and adding more product makes that one worse rather than better.

Can I just ask for a specific number of units?
You can ask, and you will get an honest answer about whether the assessment supports it. If it does, that is what happens. If it does not, we will explain why and tell you what we would do instead. What will not happen is a number being injected simply because it was requested.
